B. COUNTRY STUDIES OF ENDEMIC GOITER AND CRETINISM
1. Give brief description of each of latest country and regional studies on prevalence of endemic goiter and cretinism showing: date of the study, population studied, type of sample and its representativity, the clinical classification method used, and th e clinical and laboratory results (attach copies of the report(s) and publications referred to).
2. lf there is a system for the epidemiological surveillance of the prevalence of endemic goiter and cretinism, describe it and indicate the responsible institution, the intervals at which information is gathered, the type of information sought, the clini cal classification used, and recent results.
3. List the studies done in the country on goiter, cretinism, thyroid function and/or mental retardation, giving names of researchers, their addresses and institutions, the titles of the studies, dates, and important results (attach copies of the reports and articles cited).

E. SALT IODIZATION
1. Procedures
Briefly describe the processes by which salt is iodized in the country (dry, in brine, by
dripping, spersion, etc.), the iodine compound used (iodide, iodate, etc.), the stabilizer
added to the dine compound, the required level of iodization and the actua l level reached
in each plant, control procedures in the plant and intervals at which carried out, the
packaging, and the national and/or regional coverage.

3. Iodization Costs
Estimate the cost of iodization per metric ton of iodized salt and explain who bears those
additional costs. If possible, give the cost of the equipment. If the iodine compounds are
imported, state which, from where, at what cost, and in what quantities i n each of the
last five years, if possible. If the compounds are produced locally, try to ascertain the
quantities purchased by the iodization plants in each of the last five years, if possible.
F. MONITORING AND CONTROL OF SALT IODIZATION
1. Standards and Procedures
State the current standards and those currently enforced for the control of salt
iodization, the responsible agencies, the intervals at which the controls are practiced,
the procedures (sampling location, collection methods, laboratory techniques), the co
verage of the control program (nationwide or regional), and its cost. Also, state whether
specific standards are applied for inplant quality control and on the marketing,
packaging, transportation, and sale of iodized salt.
2. Control Activities in the Last Year (l982)
Describe the number of samples analyzed by place at which taken (a production plant, local
markets, homes, etc.) and the iodine levels found (frequencies of classes of levels in ppm
if the analyses are quantitative). Also, say what measures are taken if insufficient
iodization levels are detected.
3. Control Problems
Describe the chief problems to the systematic control of iodization at the national level,
the measures taken to solve them, and those that should be taken.
G. USE OF IODIZED OIL
If iodized oil is administered parenterally or perorally, say whether this is done sporadically or regularly, to whom, the doses administered at different ages, the compound used, the intervals at which administered, the problems encountered, and the resu lts of evaluations, if any have been made (attach copies of the evaluation reports). lf studies have been done on the peroral or parenteral administration of iodized oil, briefly describe them and give their results (attach copies of the publications and reports).
H. PROGRAMS AND ACTIVITIES FOR THE PROMOTION OF AND EDUCATION ON ENDEMIC GOITER, CRETINISM, AND THE USE OF IODIZED SALT
Describe the programs and activities, if any, for informing and motivating the general public (or professionals or other specific groups) on endemic goiter and cretinism, and the use of iodized salt. Include both education and information programs throug h mass media, and specific activities for information (seminars, conferences, courses, etc.) and for formal and informal community education.
